We seek your support to test and fully implement the Passport to Trust Program to minimize variation in care and optimize health outcomes in the United States. Funding to this program through Beth Israel Deaconess Medical Center, a major teaching hospital of Harvard Medical School and non-profit organization, is essential to bring this work to the next level allowing for potential scalability across the nation and government support.

Funding will allow us to optimize health care delivery by:

    • Structuring the patient- physician encounter to improve two-way communication
    • Creating individual Roadmaps for diagnosis and treatment
    • Educate physicians and patients in real time with access to the latest information
    • Building sustained mutual partnerships to improve patient outcomes at lower cost
